We have two ways you can apply for help. 1) You can use the form below to apply, or 2) you can email us through our contact page to have a paper form sent to you via email.

To apply you must be able to prove income, expenses, need, & proof you own pets. If you use the form below to apply we will contact you to request all needed documents and to set up a phone interview. If you apply via fax you can fax the needed forms with your completed application.

If you have recently received help from your local food pantry or trustee’s office you will then only need to show proof of help, & proof you own pets.
